Siksika Health Services Alberta
Objective: to determine the effectiveness of using a digital therapeutic solution and Remote Patient Monitoring (RPM), to prevent and reduce occurrence of diabetic foot ulcers in Siksika Nation members living with type 2 diabetes and diabetic peripheral neuropathy.
The objective of this project is to map the current reality of diabetes care at Siksika Health Services to identify inefficiencies and gaps in care. This project will be centered around gathering information to inform the re-creation of the diabetes program at Siksika Health Services. We expect the benefits will include improved care and a dramatic reduction in severe complications from diabetes. These goals will be achieved by creating stronger systems around case management, patient follow-up, and the regularization of diagnostic measures in patient care. The impact of reducing severe complications from diabetes will be felt community-wide as 15% of the community is known to be living with diabetes; every member is either directly affected by or knows someone who is affected by diabetes. Improving diabetes care will improve health outcomes and create a better quality of life for patients and their support systems.
