Objective: education and awareness about diabetes and how it can be delayed/prevented through early awareness programs to empower individuals to take responsibility and create support teams
Ebb & Flow First Nation Health Authority with provide diabetes knowledge sharing by creating a safe environment with the goal of helping those with diabetes accept their diagnosis and realize they are not alone. As part of the project, they will create a community & clinical team, strictly on diabetes support and help identify linkages to a Family Doctor, Dietitian, Pharmacist, Eye Doctor, Dentist, Footcare Nurse, Supports from Family & Friends. .
